Hospitalize all patients who present with P. falciparum malaria.  |
- Hospitalize all patients, especially children, pregnant women, and the elderly, with malaria, regardless of their age or clinical status.
- Do not make assumptions about patients' immunity or clinical course based on their previous history of residing in a malaria-endemic area.
- Admit all patients with severe manifestations of malaria to an intensive care unit, where the following interventions are readily available:
- Continuous ECG monitoring during quinidine infusion
- Mechanical ventilation
- Continuous hemodynamic monitoring
- Hemodialysis
- Monitor pulmonary, renal, and neurologic status as for acutely ill patients in the ICU setting.
- Monitor serum glucose every 4 to 6 hours during quinidine infusion and with any change in neurologic status.
- Monitor parasitemias every 6 to 8 hours until negative.
