Provide appropriate hydration and administer uric acid-lowering drugs to patients receiving chemotherapy for hematologic cancer who will have substantial tumor lysis. A

  • Start allopurinol therapy (600 mg on day 1 and adjust dose for renal function to 100-300 mg/d) during chemotherapy for hematologic cancer, such as acute leukemias or high-grade lymphomas, unless the patient is known to be allergic to allopurinol.
  • Note that some guidelines have recommended rasburicase, 0.1 to 0.2 mg/kg·d for 1 to 7 days, for tumors associated with a high risk of tumor lysis syndrome.
